Flat Foot (Pes Planus)
If the arches on the inside of your feet are flattened, it means you have flat feet. This condition allows the entire sole of your feet to touch the surface of the floor whenever you stand. Though, the flat foot is considered as a common disorder, yet it comes with complex deformity of the hindfoot as well as forefoot. You may also call flat foot deformity as posterior tibial tendon dysfunction, pes planus, or fallen arches.
Congenital pes planus deformity can occur when the arches don’t develop properly during childhood. In other cases, it can also happen due to injury or from simple wear and tear age stress. You need not to go for any kind of treatment until you are observing any pain, but in any case, if you are feeling discomfort, then make sure you are opting for Flat foot treatment from orthopedic doctor.
- A flat foot is normal in toddlers and infants because the foot’s arch does not develop. Though in most, arches develop as the child grows, yet some people may not develop arches throughout their life.
- Accessory navicular bone (Os naviculare) is an abnormal bone in the inner side of the foot which is frequently associated with a flat foot deformity in young adults
- Tarsal coalition is a rare condition characterized by an abnormal connection between two bones in the foot and it may also flat foot in children/ young adults.
- Posterior tibial tendon dysfunction (PTTD): Years of wear & tear weaken the posterior tibial tendon; it is considered as the main cause of Flat foot in adults.
- Generally, flat foot symptoms are minimal to none but some patients may observe pain inside the ankle and on foot outside or just below the ankle.
- Foot fatigue may be the first sign of flat foot problem.
- Excessive wear out of shoes from inside.
- Difficulty in rising up on your toes
- Over time, flattening of arch causes the heel to tilt outwards (valgus) and the forefoot to rotate outwards (abduction)
- This can also predispose patient to bunion, shin splints and aching of lower back, knee, and hip.
The treatment of Flatfoot depends upon its stage of progression, type, or symptoms. Detailed evaluation by a foot and ankle specialist is needed to differentiate between physiological and pathological flatfoot and between flexiblepes planus and rigid. It is advisable to go for early treatment, no matter your condition is flexible or rigid.
Most of the cases of flat foot can be treated by flat foot insoles or medial arch support customized to the patient’s foot. Some muscle training exercises are also helpful in cases with weak muscles. However, if the deformity is too advanced or the non-operative treatment does not work, surgery is needed in such cases. Flatfoot surgery needs a lot of planning as well as decision making in view of complexity of the deformity.
